Referral Process
Submit Referral
Use the online referral form or email our intake team. Attach the assessment and treatment plan if you have them.
We Review & Contact
We verify eligibility and insurance, then contact you within 24–48 business hours with next steps.
Client Onboarding
We complete the functional assessment, build the treatment plan, and begin services.
What We Need
- Client name and contact information
- Diagnosis and functional impairment areas
- Current treatment plan, if available
- Insurance information (MA plan)
- Your contact information
- Documented client consent to refer
Turnaround Time
We aim to respond to every referral within 24–48 business hours.
For urgent or crisis referrals, call (612) 000-0000 and flag the referral as urgent on the form.
